COPE addresses the chronic healthcare diagnoses and disease management needs of individuals and their communities. Our trans-disciplinary approach integrates community outreach, healthcare access, health literacy, and engagement services, as well as social and healthcare provider capacity building and health policy advocacy. We work across sectors and stakeholder groups to foster an environment of care that reduces health disparities and achieves optimal health outcomes.
The Cornerstone of COPE’s Approach
Peer-Community Health Workers are the cornerstone of COPE’s core services of community outreach and testing, health literacy education, and grassroots behavior change interventions. Building rapport and trusting relationships with our clients are the cornerstones of all we do.
Because our peer community health workers have personally known many of the challenges faced by our clients, they are able to build rapport and trusting relationships with them. They are experts in being able to work and walk with clients to ensure access and engagement in care. Working alongside our patients and provider partners in health care and social services, our clients develop the trust, skills, and confidence needed for strong patient-provider relationships and streamlined systems of care that support the provision of holistic and coordinated services.
COPE’s approach is trauma-informed and rooted in a harm reduction approach for engagement in healthcare.
